Nurse Practitioner-Led Clinics In Ontario

Over the next three years, Ontario's Family Health Care for All Strategy is focusing on continuing to improve access to family health care and reduce the number of unattached patients through the implementation of 25 Nurse Practitioner-Led Clinics and 50 new Family Health Teams.

Targeted to open by 2011/12, the 25 Nurse Practitioner-Led interdisciplinary teams will include nurse practitioners, family physicians, and other providers such as nurses, social workers and dieticians all working together to see more patients and keep them healthy.

Nurse Practitioner-Led Clinics will be focusing on delivering comprehensive primary care services to unattached patients and will offer additional programs tailored to the needs of their communities. Like Family Health Teams, they will support increasing chronic disease management and prevention including the Ontario's Diabetes Strategy.
