17 More Teams Invited to Complete Full Application to Become an Ontario Health Team

As part of the province's plan to end hallway health care, Ontario Health Teams strengthen local services, making it easier for patients to navigate the system and transition between health care providers.

Under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team - no matter where they provide care.

There are currently 29 Ontario Health Teams established across the province. A further 17 teams are now being invited to move on to the next phase of the readiness assessment process and complete full applications to become an Ontario Health Team.

If approved and at maturity, these 17 teams will join the already approved 29 teams to cover 80 per cent of the province's population. Ontario will continue working with all health system providers until full provincial coverage is achieved and everyone in Ontario is supported by an Ontario Health Team.

The 17 teams now invited to complete a full application to become an Ontario Health Team are:

  • Brantford Brant Team
  • Downtown East Toronto Team
  • Kawartha Lakes Team
  • Elgin Team
  • Grey-Bruce Team
  • Connected for Care - Lanark, Leeds and Grenville Team  
  • Kitchener, Waterloo, Wellesley, Wilmot and Woolwich (KW4) Team
  • Mid-West Toronto Team
  • North Simcoe Team
  • Oxford and Area Team serving Oxford Region - Ingersoll, Woodstock and Tillsonburg

  • Rainy River District Team
  • Frontenac, Lennox and Addington Team serving Kingston, Greater Napanee and surrounding areas
  • Sarnia Lambton Team serving Lambton County
  • Scarborough Team
  • South Georgian Bay Team serving Collingwood, Stayner, Wasaga Beach, Creemore, Clearview and Clarksburg
  • West Toronto Team
  • Windsor Essex Team 

Ontario will continue to support teams as they work towards completing their full application.
